Dr. Thomas Shireman

The Catalyst in Disguise

🧾 Role Summary

 

Dr. Thomas Shireman was the GI specialist who performed Nicole's ERCP on September 3, 2019, the day everything began to unravel. His name appears briefly in the records — just enough to show he was there — before vanishing entirely from the disaster he helped set in motion.

Shireman performed the procedure despite Nicole having no gallbladder and no stone, and allegedly placed a bile duct stent. Nicole clearly recalls being informed of this post-op. In the immediate aftermath, she suffered two STAT events — now believed to be lipid emboli — and began vomiting bile daily, a symptom that would haunt her for over a year.

No complications were listed in the surgical notes.
No stent was ever officially acknowledged again.
But the evidence never went away.

🔍 The Smoking Evidence

 

  • September 10, 2019 MRCP:
     
    • Shows a focal bile duct stricture dismissed as “artifact”
       
    • Reveals liver enlargement (19.8 cm)
       
    • Ordered specifically for “recent stent placement”
       
  • Nicole’s lived experience:
     
    • Remembers being told a stent was placed
       
    • Bile vomiting began immediately after the procedure
       
    • Symptoms resolved only after a second, unrelated ERCP in November 2020
       
  • Doctors referenced a stent in subsequent notes — but later denied it ever existed
     

Despite the physical signs and the circumstantial confirmations, Shireman never reappears in the record, never followed up, and never accounted for the harm his procedure caused.

📉 Timeline of Failures

 

  • September 3, 2019 – Performs ERCP; alleged stent placed. No complication recorded. Nicole suffers two STAT events hours later.
     
  • September 4–10, 2019 – Begins bile vomiting. Condition declines. No surgical follow-up from Shireman.
     
  • September 10 – MRCP shows stricture. Radiologist references “artifact,” omitting discussion of prior ERCP or stent presence.
     
  • Following months – Every mention of the stent disappears from narrative. Shireman vanishes entirely from the clinical arc.

💬 Commentary

 

“You don’t need to falsify records if you just don’t write anything at all.”
 

Dr. Shireman didn’t have to lie.
He just had to leave enough blank space for others to do it for him.
No follow-up. No clarification. No accountability.

He placed the match.
Then he walked away from the fire.
